Liability Waiver

This Release Agreement applies to Stick N Puck Hockey Training Ltd. d.b.a. Atomic Hockey, its directors, officers, employees, coaches, contractors, volunteers, representatives, successors, and assigns (collectively, the “Releasees”).
 “Facility” refers to the Atomic Hockey facility in Langley, BC, as well as any off-site locations, including arenas or rinks where Atomic Hockey conducts programming.

You must be 19 years or older to sign this form, or signing on behalf of a minor as a parent/legal guardian/coach.

I acknowledge that participation in Atomic Hockey programs — which may include, but are not limited to, on-ice training, synthetic ice training, skatemill sessions, shooting lanes, off-ice conditioning, use of specialized or motorized equipment, resistance training, and training aids — involves inherent and foreseeable risks, including but not limited to:

I understand that all facilities, surfaces, training tools, and environments carry risk, and these risks may not be fully eliminated even with proper supervision and safety protocols.

I voluntarily choose to participate in Atomic Hockey activities and fully assume all risks, both known and unknown, associated with my participation or the participation of the minor in my care.

I acknowledge and agree that any personal property I bring to the Facility or any off-site location is brought at my own risk. The Releasees are not responsible for loss, theft, or damage to personal belongings.

I hereby waive, release, and forever discharge the Releasees from any and all claims, demands, actions, damages, losses, expenses, or liabilities arising from or related to:

This waiver applies whether the claim arises before, during, or after participation.

I agree to defend, indemnify, and hold harmless the Releasees from any liability, claims, damages, legal fees, or costs arising from:

I agree to pay for any damage I (or the minor in my care) cause to Atomic Hockey property, equipment, or facilities, whether intentional or accidental.

I agree to follow all instructions and safety rules provided by Atomic Hockey staff.
 I understand that:

I confirm that I (or the minor in my care) am physically capable of participating in Atomic Hockey activities.
I have disclosed any relevant medical or physical conditions. I understand Atomic Hockey is not responsible for medical incidents or emergencies.
I authorize Atomic Hockey staff to seek or arrange emergency medical care as deemed necessary, and I accept full financial responsibility for any associated costs.

I understand that Atomic Hockey is not required to issue refunds, credits, transfers, or proration for missed or unused sessions due to injury, illness, scheduling conflicts, or any personal circumstances.

I grant Atomic Hockey permission to photograph, record, or film my participation (or that of the minor) and to use such media in training, marketing, promotional, and educational materials without compensation or restriction.
If you require a separate opt-out, inform staff in writing.

This Agreement is governed by the laws of the Province of British Columbia.
 Any disputes will be resolved exclusively in the courts of British Columbia.

If any portion of this Agreement is found unenforceable, the remaining sections shall remain in full force and effect.
 This Agreement constitutes the entire understanding between the parties and supersedes any prior representations, written or verbal.
